Q&A Brain "insults"

When brain insults are mentioned as a diagnosis without the term infarct, what is the best icd9?

Radiologist's will often report 70540 and 70544 together with an indication of headache, dizziness, etc. In the body of the report they mention as an example " cerebral insults " they will usually mention types of "ischemias" chronic or age indeterminate. Since there is no mention of an "infarct", I struggle with decoding the language and selecting the best icd 9. I am uncertain if 434.90, 435.8 or 437.1 are the best choices and if so which one accurately describes what has taken place? Also, with icd 10 will there be better choices for the word "insult"?
