Q&A E/M during Post-Op Period (GSP)

Question: Provider billed with modifier 24 with an ICD-9 for trigger thumb only. Does this justify the 24? Is this significant enough to warrant payment of E/M within global?

Patient is seen by orthopedist 60 days post shoulder arthroscopy. Doctor reviews progress of shoulder as well as related physical therapy. He also mentions that she had a trigger thumb injected 16 months prior. States it is starting to bother her again.

Exam of shoulder includes flexion and abduction and internal and external rotation. Triggering is noted in the right thumb.

Assessment is documented as the following: (1) S/P R shoulder arthroscopy and open rotator cuff repair utilizing 2 bio swivel locks, acromioplasty, 09__/15. (2) R trigger thumb injection 9/2014

Plan: Off work status is addressed concerning s/p shoulder arthroscopy. Restrictions given.

R thumb triggering discussed. Opinions reviewed. Daypro prescribed with documentation that states it will help the thumb as well as shoulder pain. Follow-up in one month. If triggering not better than repeat cortisone injection will be done.
