ICD Patient given wrong test

A colleague came across an ER visit that a CT scan WITHOUT contrast was ordered for a patient who recently had a kidney transplant due to right sided abdominal pain. Unfortunately, the patient was given contrast and was treated with IV fluids. There were no adverse reactions. The big question is, Would you use a poisoning code for the contrast? 977.8 And would you use E947.8 (contrast media) or E876.8 other specified misadventures during medical care? Yeah, I know, it's a toughie. Any help would be greatly appreciated! :)

That is a toughie. I believe the way you want to code it is correct. I was thinking "misadventure" but I think that would have to be documented as such. Here is a link I found interesting

Thank you Laureen! Misadventure was NOT documented, so we are going to go with E947.8
