Verizon as an email provider has recently started blocking all email that is received from Nanacast, our eCommerce provider.  Nanacast has tried to contact Verizon multiple times to clear the block, but have received no response or assistance from them.

So, if Nanacast allows you to purchase with a Verizon email, you would not receive an order receipt, login instructions, billing updates or other order-related emails. Naturally, this would cause significant frustration with Nanacast customers, so they decided to block all Verizon emails from purchase. 

Nanacast will restore access as soon as Verizon removes their block. Until then, it’s not safe to register with Verizon emails. Please use another email address to purchase.

What Exactly Is Risk Adjustment / HCC Coding?


In brief risk adjustment coding is done using a special code set, by auditing a previous year’s worth of an individual’s medical documentation to confirm chronic conditions so that a dollar amount can be predicted for that patient’s care in the future year.

Due to it’s success with Medicare many private payers are using this method as well causing and increase in demand for Risk Adjustment coders. So much so that many companies will hire newly certified coders and even provide initial addition training to get them up to speed.

Continuing education units are required for certified coders to show they are staying current on coding information.

  • If CIRCC®, 16 must be related to interventional radiology or cardiovascular coding
  • If CPMA®, 16 must be related to auditing
  • If CPCO™, 16 must be related to compliance
  • If CPPM®, 36 must be related to practice management
  • If a specialty, 8 must be specialty specific

Two Certifications:

40 CEUs every two years*

Three Certifications:

44 CEUs every two years*

Four Certifications:

48 CEUs every two years*

Five or More Certifications:

52 CEUs every two years*

*Specific Credential Requirements:

Note: Events must be specifically pre-approved for CIRCC®, CPMA®, CPCO™, CPPM®, and all specialty credentials, based on relevance and level of difficulty, in order to be eligible for CEUs.

OnceCIRCC, CPMA, CPCO, CPPM and specialty CEUs have been met, the remaining credits due can be obtained from any pre-approved program.

If CEUs have been pro-rated due to a corporate membership addition, this may exceed the cap of 80 CEUs every two years.

Effective October 1, 2012, pre-approved Specialty CEUs can be applied toward one Specialty and one Core credential concurrently.

**Pay membership every year and submit your CEUs online via the CEU tracker every other year. **

CEUs are reported every two years on or before your membership due date. You must pay your dues every year, even on a year when you are not required to report CEUs. The year that your CEUs are due is based on the month of your membership*:

If your membership is due in the months below, two years of CEUs are due on your membership expiration date in 2015

  • January
  • March
  • May
  • July
  • September
  • November

If your membership is due in the months below, two years of CEUs are due on your membership expiration date in 2016

  • February
  • April
  • June
  • August
  • October
  • December

*If you are newly certified, CEUs will likely be pro-rated to match your existing renewal month for your first submission. CEUs earned prior to certification will not be accepted.

  • CEUs must come from the AAPC Approved vendor list.
  • CEUs must be earned and submitted within your two-year renewal period*. Excess CEUs do not carry over to the next renewal period.
  • CEUs earned within your renewal month can apply to either the current submission or the next
  • AAPC awards all CEUs based on completion date except for Healthcare Business Monthly Test Yourself quizzes, which are awarded based on publication date (be sure to only use issues from your specific timeframe).

AAPC is approving this because specialty approved CEUs are also approved for core credentials. This should save members time and money in the future. The CEU Tracker will be modified to easily enable the recording of CEUs.

At this time, CCO is only AAPC approved for CEU’s. CCO does not pay AHIMA or other certifying agencies to get our CEUs pre-approved. We suggest that you contact AHIMA or other certifying agency & ask them whether you can use the CCO Q&A Webinar CEU towards their CEU requirements. What year’s books should I use?

Since the code sets for each coding book are updated every year, it’s essential for coders to have the current year’s books to ensure accurate performance. Reporting wrong codes can lead to denied claim submissions, and substantial penalization. It’s clearly very important to get an education on the annual code set updates, and to obtain new code books every year.

All AAPC exams are based on the current calendar year’s code sets, so we strongly suggest that you use the current year’s books. The previous calendar year’s books may be used on an exam, but you would be at obvious disadvantage doing so. The upcoming year’s books are not allowed for exam use. The exams are updated every January for the new code sets. Examinees using their second attempt in the next calendar year will not be permitted to take the previous year’s exam.

This is a very elementary tip, but it is surprisingly unknown and under-utilised, so here is a basic guide for some extra functionality in Google searches.

It’s useful when:

• a site lacks a good search engine

• when you are more familiar with Google search

• when you wish to search an entire site or sub-directories

• when you wish to find files of a certain type on a site

• getting an overview of the number of pages or items (such as images) on a site.

“site colon” – get an overview of the number, or types of pages on a site.

In a standard google search box, type:

“site:” + [root URL of the site you want to explore]

“Root URL” means the web address without http:// and/or www.

eg. site:cco.us.com

“site colon” + images – see all images on a website

In a standard google search box, type:

“site colon” query

“site:” + [root URL] + [space] + [query]

= every reference to query on the website.

eg. [site:cco.us.com %s] then you’ve got every reference to %s on cco.

Click the images link (for google images), and you’ll see all images on the site.

  1. Do a site: search for an unusual distinctive character eg.∆
  2. Copy the URL
  3. In your browser’s options (settings, preferences) find it’s search engines.
  4. Paste the URL in the appropriate place and replace your character with the required character (eg. %s for Chrome)
  5. Set a shortcut for the search using a sequence of characters that you know you probably won’t search for (eg. scsc)
  6. When you find a site that you wish to search, copy it’s root URL.
  7. Open a new page or tab.
  8. Type your character. It sequence will immediately switch to a site: search engine.
  9. Paste in the URL (with or without a search term separated by a space) and hit enter.

Medical coding is the process of reading a healthcare provider’s documentation to identify what was diagnosed, and the corresponding procedures and services that a healthcare professional performed, and recording this information in an established code format, which the coder has learned. The codes are entered into a computer system, and are nationally recognized by other health care professionals to accurately reflect the patient’s case.

A medical biller takes those codes, and submits the insurance claim to the patient’s insurance company for payment of services rendered by a healthcare provider. The medical biller must have billing knowledge, so this can be done properly. In some cases, one person may be both a coder and biller, but for larger facilities, the jobs are usually fulfilled by different people/departments.

Career Schools

Career schools, also known as technical, vocational, or trade schools

  • may be public or private, although many are for-profit businesses;
  • typically offer programs that are two years or less; and
  • provide students with formal classes and hands-on experience related to their future career interests, from welding to cosmetology to medical imaging.

The previous calendar year’s books may be used on an exam, but you would be at obvious disadvantage doing so. The upcoming year’s books are not allowed for exam use. The exams are updated every January for the new code sets. Examinees using their second attempt in the next calendar year will not be permitted to take the previous year's exam.

What is allowed in our books?

Handwritten notes are acceptable in the coding books only if they pertain to daily coding activities. Questions from the Study Guides, Practice Exams or the Exam itself are prohibited. Tabs may be inserted, taped, pasted, glued, or stapled in the manuals so long as the obvious intent of the tab is to earmark a page with words or numbers, not supplement information in the book.

No materials (other than tab dividers) may be inserted, taped, pasted, glued, or stapled in the manuals.

If you are a certified coder then you may want to use a coding tool, at least for some of your charts.

Will Computers Replace Human Coders?

As CAC technology becomes increasingly sophisticated there will be less demand for coders to perform manual coding tasks. Computers will not replace all of the people who are currently working as clinical coders, but computers will begin to reduce the number of hours spent manually assigning codes. Computers are not capable of taking on the new roles and responsibilities and performing the review, validation, and oversight tasks that will be created as a result of computerization of clinical coding.

Just as software applications have continued to slowly evolve over the past several decades to create tools that assist transcriptionists (versus replacing them), CAC technology should be viewed as a tool to assist coding staff rather than as a replacement for coding staff. Though it is anticipated that computers will take over some coding tasks, computers are not expected to replace human coders. Just as transcriptionists who work with the latest technology (e.g., speech recognition) have modified their role to become “expert editors,” automation tools for coding will likely result in a role change for coding professionals and will result in the better use of such staff for complex decision support tasks.

It should be noted that there may be some circumstances where CAC can be applied without human intervention today. Users reported to the work group limited instances where confidence in the CAC code output was high and only random editing was performed. An example of this is code assignment for normal mammogram reports. As these systems advance, the range of which situations will be acceptable for direct computer-generated coding is expected to increase. Overall, however, CAC, without human review, is not to the point where large displacement of the coding work force can occur to any significant degree.

Short Answer:

Refunds are not an option for extensions. We do offer a 7 day refund on our foundation courses. However, when a person signs up for the extension, a stipulation is that the student agrees that they are responsible to cancel the subscription when they no longer need it.

Rationale:

We offer extensions at a cost-effective rate making it available to the student who has already paid for the year, but needs a little extra time. We leave it up to the student to decide when they feel confident to step away from the learning process and move toward certification. We can not make that decision.

We can offer this low rate to extend monthly precisely because we don’t have to monitor the extensions, or perform any administrative work (such as refunds). The responsibility is solely on the student to monitor their extension duration, and cancel it when they no longer need the material. Q. Do I need to create an account?

Yes, you must create an account before submitting a question to the AHA Central Office. The AHA Central Office needs complete contact information in order to request additional information and to send the official response to you.

Q. I can’t find my tracking number. How do I request it?

Click Request My Tracking Number located at the bottom left corner of the home page. You will need to provide the following information: approximate timeframe question was submitted, method of submission (e.g., mail), type of question (e.g., ICD-9-CM) and brief description. With that information, we will search the database and e-mail the tracking number that best matches the information provided.

Q. Can I submit multiple questions at one time?

Q. What types of supporting documents should be included as part of the question submission?

The AHA Central Office will not respond to the following HCPCS codes:

  • Inquiries from physician providers related to CPT-4. These questions will be referred to the American Medical Association (AMA).
  • Questions related to HCPCS level II codes except for A-codes, for ambulance services and radiopharmaceuticals, C-codes, G-codes, J-codes and Q-codes other than Q0136 through Q0181.

We regret that the AHA Central Office clearinghouse is not able to support requests for ICD-10-PCS coding advice related to outpatient procedures. Such requests for coding advice will be returned unanswered. However, we are happy to assist you with HCPCS coding questions for outpatient procedures. The ICD-10-PCS code set was not designed to support the coding of most services that are performed by providers in the outpatient setting. The Health Insurance Portability and Accountability Act (HIPAA) Administrative Simplification Modifications to Medical Data Code Set Standards (published in the Federal Register, Vol. 74, No. 11, pp. 3328-3362, January 16, 2009) adopted ICD–10–PCS for inpatient hospital procedure coding only.

The National Correct Coding Initiative (**NCCI**) contains two types of edits: NCCI procedure-to-procedure (PTP) edits and Medically Unlikely Edits (MUEs).

Types of NCCI Edits

The National Correct Coding Initiative (NCCI) contains two types of edits:

  1. NCCI procedure-to-procedure (PTP) edits that define pairs of Healthcare Common Procedure Coding System (HCPCS) / Current Procedural Terminology (CPT) codes that should not be reported together for a variety of reasons. The purpose of the PTP edits is to prevent improper payments when incorrect code combinations are reported.
  2. Medically Unlikely Edits (MUEs) define for each HCPCS / CPT code the maximum units of service (UOS) that a provider would report under most circumstances for a single beneficiary on a single date of service.

The CMS National Correct Coding Initiative (NCCI) promotes national correct coding methodologies and reduces improper coding, which may result in inappropriate payments of Medicare Part B claims and Medicaid claims.

National Correct Coding Initiative

The CMS developed the National Correct Coding Initiative (NCCI) to promote national correct coding methodologies and to control improper coding leading to inappropriate payment in Part B claims. The CMS developed its coding policies based on coding conventions defined in the American Medical Association’s CPT Manual, national and local policies and edits, coding guidelines developed by national societies, analysis of standard medical and surgical practices, and a review of current coding practices. The CMS annually updates the National Correct Coding Initiative Coding Policy Manual for Medicare Services (Coding Policy Manual). The Coding Policy Manual should be utilized by carriers and FIs as a general reference tool that explains the rationale for NCCI edits.

Carriers implemented NCCI Procedure-to-Procedure(PTP) edits within their claim processing systems for dates of service on or after January 1, 1996 and began implementing Medically Unlikely Edits (MUE) on January 1, 2007.

A corresponding set of PTP edits is incorporated into the outpatient code editor (OCE) for OPPS and therapy providers [Part B Skilled nursing facilities (SNFs), comprehensive outpatient rehabilitation facilities (CORFs), outpatient physical therapy and speech-language pathology providers (OPTs), and certain claims for home health agencies (HHAs) billing under TOBs 22X, 23X, 75X, 74X, 34X]. Corresponding MUE are similarly implemented within the Fiscal Intermediary Shared System (FISS).

The purpose of the NCCI Procedure-to-Procedure (PTP) edits is to prevent improper payment when incorrect code combinations are reported. The NCCI contains one table of edits for physicians/practitioners and one table of edits for outpatient hospital services. The Column One/Column Two Correct Coding Edits table and the Mutually Exclusive Edits table have been combined into one table and include PTP code pairs that should not be reported together for a number of reasons explained in the Coding Policy Manual. The purpose of the NCCI MUE program is to prevent improper payments when services are reported with incorrect units of service.

Funds availability details

When some sellers receive payments, we may hold the money in their pending balance for up to 21 days. We want to make sure that there are no problems with the orders, such as disputes, claims, returns, or charge-backs. After we determine that the order was fulfilled and the customer is satisfied, we may release the money earlier.

If we place your money in a pending balance, it belongs to you but isn’t immediately available to spend or withdraw.

We’ll let you know if payments on your account are held. We’ll also let you know what steps you can take to have your money released earlier. If your PayPal account was already limited, you’ll need to lift those limits before you complete the steps to release your payments.

How does it work?

Why aren’t my payments available right away?

Delaying the availability of payments for a specified amount of time is a common practice in the payments industry. Delaying access to payments helps make sure that sellers have enough money in their account to cover claims or refunds.

When does PayPal release the payment?

If there are no problems with a transaction, the money will typically be released within 21 days after the buyer pays. If there are problems, such as a dispute, claim, charge-back, or return, we’ll release the payment when the issue is resolved. After you fulfill the order and mark it as processed, typically the payment will be released sooner.

The money may be released early if:

  • You ship directly using USPS, UPS, or FedEx, or pay for shipping and print labels on PayPal. Once we know that the item was delivered, we’ll release the payment within 3 days, OR
  • You mark an order as shipped or processed. (This applies even if you don’t provide tracking information through USPS, UPS, or FedEx or if the order does not require shipping.) We’ll calculate the expected delivery date and release the payment within 7 days of that date, OR
  • You mark the item as processed and we know that the buyer has received it and is happy with the item. We’ll release the payment within 7 days.

 How is the estimated delivery date calculated?

After you mark your order as shipped or processed, we calculate the estimated delivery date using a combination of the processing date, the type of service or shipping company, handling time, and the shipping address, if applicable. For orders which don’t require shipment, we determine the appropriate handling time based on the information you provide.

Can I pay for shipping with money in my pending balance?

When you print labels and pay for shipping through PayPal or eBay, the cost of shipping is released from your pending balance shortly after purchase. Printing labels on PayPal and eBay is free. You pay only the cost of shipping the item.

What can I do to avoid delayed payment?

Here are a few simple steps you can take to make sure that your money is available sooner and that future payments aren’t delayed:

  • Process orders right away and ship quickly using a service that provides tracking and delivery confirmation.
  • Upload or save tracking information or Delivery Confirmation numbers.
  • Communicate early and often with your buyers.
  • Confirm the address and phone numbers associated with your PayPal account, and keep them up to date.
  • Verify your account and keep your financial information (like bank account and credit card numbers) up to date.
  • Avoid buyer disputes, claims, and charge-backs.
  • If you have a buyer dispute, resolve it quickly.
  • If you sell on eBay, improve your Detailed Seller Ratings (DSRs) scores.

Who is affected by payment delays?

Payment delays typically affect new sellers or sellers in categories that have a higher than average rate of refunds, disputes, or charge-backs.

In deciding whether to hold payments in a pending balance, we review many factors. These factors include transaction activity, the rate of customer disputes, the type of business, average delivery time, customer satisfaction, and history. We understand that some experienced sellers may be affected. While history with PayPal is important, it’s only one of many factors we consider.

Here are some common reasons we may hold payments in a pending balance:

  • You have limited history or selling activity.
  • You have low Detailed Seller Ratings, negative Feedback, or other indication of below-standard performance on eBay.
  • You have a high rate of customer refunds, disputes, claims, or charge-backs.
  • You're selling in a high risk category or industry such as tickets, travel, gift certificates, computers, consumer electronics, or mobile phones.
  • Your business or selling activity is inconsistent. For example, you have a spike in selling activity, your average selling price changes, or you started selling in a new category.
  • The account information you have provided is incomplete or inaccurate, or we're uncertain about the information you've provided.
  • Your withdrawal activity has changed.

An “open book examination” is one in which examinees are allowed to consult their class notes, textbooks, and other approved material while answering questions.

Two Types of Open Book Examinations:

One may think of two kinds of open book examinations, say the restricted type and the unrestricted type.

In the restricted type of open book examinations, students are permitted to bring into the examination room one or more specific documents approved by the course instructor.

In the unrestricted type of open book examinations, students are free to bring whatever they like.

Why can’t I get into my Webinar / Meeting / Training?

When using Internet Explorer to start a Meeting or Webinar, the Citrix Launcher may begin to execute and then drop off, returning the user to the My Meetings, My Webinars or My Computers page without further action.

This is due to an interference that can usually be overridden by adjusting one of the following options (PC Only):

7. If I am kicked off the webinar, will I be able to join again?

8. How do I submit my CEU to AAPC?

9. Are these webinars approved for CEU’s from AHIMA yet or will they be in the future?

It is something we keep monitoring, but for now the cost to CCO is prohibitive given that we do not charge for the webinar. However, you can write to AHIMA and list the topics covered, and ask them if they will accept the AAPC CEU towards AHIMA credit. We were told that this is done on an individual basis.

10. Can you earn CEUs after finishing a course and waiting to sit for the CPC exam?

You must be certified to earn CEU credits. If you are not yet certified, then no, you must wait until you are certified.

11. During the webinar it says I am muted by an organizer. Why?

With 500 or more people attending the webinar, it is necessary to mute all attendees, so that we can hear the speaker with minimal background noise.

12. Do you have a hashtag associated with CCO’s Q&A webinars?

A GUIDE TO USING THE MINI-MOCK PRACTICE EXAMS

Overview

When you purchase a single mini-mock practice exam (A-Ruby or B-Sapphire or C-Topaz) or the package of all 3 practice exams, you will get access to both the ICD9 and ICD10 versions in the membership area. For each type of exam (ICD9 or ICD10), you will have the opportunity to download a printable PDF of the exam.

It is suggested that you download the printable version and print it. Complete the Printable version of the Exam – in a timed fashion. Then, take the online version by popping in your answers to get your results.

Printable version

Number of questions: 60

Time limit: None, but the recommended time to complete the PRINTABLE PDF is 2 hours. This is to mimic the time limit of 5 hours 40 minutes for 150 questions for the real CPC Board exam, allowing some extra time to check your answers on the real exam. This helps you build the necessary speed to manage your time appropriately.

Online version

Taking the exam/ Popping in your answers

When you start this test, you are asked to enter your name, email address, and a password. Make a note of your password… you may need it later if you use the save and finish later function, aka save and resume.

It is recommended that you schedule enough time to complete entering your answers so you do not need to use the save and resume function.

Number of questions: 60 (in the same order as the Printable version).

Passing grade: 70%

Questions displayed per page: 10

You may go back and change your answers to previously answered questions.

You will not be able to submit your test with any questions unanswered.

There is a “save and finish later” function if you cannot finish in one session. Find the link for this function at the bottom of each page of 10 questions. To resume later where you left off, click on the test from your membership area, and be sure to use the Resume link (and not the “Start a new test” link). Use the same email address and password that you entered previously.

Reviewing a question: Scroll down to the bottom of any page (group of 10 questions). Click the link to display previously reviewed questions. You can go back to that question and change your answer if you wish. You will then have to keep hitting the NEXT/ PREVIEW button to get back to where you were previously in the test, or to another question that you want to review.

After you submit your answers

When you submit your answers, you will see your result and score. If you passed, you can use the “download certificate” icon to download a Certificate of Completions, which will have your name, name of test, and your score.

IMPORTANT: To see the questions, answer choices, and rationales, scroll down. Note that you cannot right click and use the Print link to print out information from this page. However, to save your score, results, marked up questions and rationales to another document, highlight (select) whatever you want to keep for later, copy and paste it into another document (e.g. Word doc) on your computer. Do NOT close the result page window until you have saved anything you want to keep!

After you exit the results page

Email: You will receive an email from CCO with a link to a ticket on the helpdesk. This ticket will have the test name and your score. Note: The link to “Results,” which is given on the ticket when it is not currently working due to a software problem. Until the software problem is fixed, you can only get the feedback from the test at the time immediately after submitting your answers. (Read the entire section above on “After you submit your answers” for more information.)

b. The rationales to each question can be seen by scrolling. We recommend that you leave this window open until you are finished reviewing the explanations as there is no way to save or print.

c. Printing and copying the test or explanations is disabled in order to protect our proprietary test and intellectual property.

4. CAN I TAKE THE TEST AGAIN

a. You can also take the test multiple times but they will be the same questions.

5. WHAT IF I DISAGREE WITH AN ANSWER

a. If you have a question about a test answer, please take a screenshot (e.g. using Snipping Tool which comes with Windows or use Jing (free download for Windows or Mac) and send it to http://www.cco.us/cco-helpdesk-request/. Email the screenshot, along with the name of the test, so that we can resolve the issue.

6. TIMING- example

What if my credit card is declined or “cannot be validated” when ordering?

If you are trying to order , here are the easy points to check:

  • We only take Visa, MC, at this time.
  • AVS mismatch – Check your billing address, it must be the same as listed on your billing statement .
  • Check your CVV2 code (the three-digit security code, usually found on the back of your card).
  • Check your credit card expiration date.
  • Some non-US banks do not support our credit-card fraud-detection process.
  • Try submitting your order again.

If it continues to happen and you have another credit card you can use, that may be the path of least resistance.

If you don’t, and you’ve confirmed all of the information above, and you have read the further explanations given on this page, you should contact your bank or credit provider to see if there is a problem.

Why Did This Credit Card Fail To Authorize?

When you make a credit card authorize call to save a credit card for future use, we run all the same checks as an actual payment (with the exception of checking for an available balance). You can think of this like making a payment for $0.

If you receive an error stating that we failed to authorize the card, this means the credit card issuer declined the authorization. The most common reasons for failure include:

  • Address verification (AVS) mismatch
  • Invalid CVV/Expiration
  • Card is flagged as lost or stolen
  • Risk or fraud decisions from the issuing bank

When a card fails authorization, we recommend having the payer verify the card information they entered, and if that is all correct, having them reach out to their card issuer to verify their billing information and attempt the payment again.

Failure Code: 01

Why am I getting a Payment Authorization failure message?

Possibilities are:

  1. Your credit card number, expiration date or verification code are not entered correctly. The verification code (or CVV) is a physical signature for the actual credit card you are holding and it will change when you receive a new card (with a new expiration date, etc.). Please make sure you enter the correct CVV or verification code for your new card.
  2. Your address is not entered exactly as it appears in your billing statements. You should contact your credit card company to make sure all addresses associated with your account are correct.
  3. Your credit card company does not support the Address Verification System (AVS). CCO requires all credit cards to support this.

If you are unable to place a transaction after confirming all of the information above, please do the following:

  1. Log into your CCO Account and click on “My Account.”
  2. Click “Credit and Debit Card” under Account Services.
  3. Remove the non-working credit card by clicking on the “Remove” button.
  4. On your web browser, delete all cookies and temporary internet files.
  5. After the card has been removed, click on the “Add” button and re-enter the card.

If you are still unable to complete a credit card transaction, please try using a different credit card.

What do I do if the credit card authorization failed for my order?

If you ordered , but we were unable to obtain a credit card authorization, something will need updating so we can complete your order. Note that, for your protection, we do not have access to the specific reason your credit card authorization failed.

The most common reasons for a failed credit card authorization are:

  • Your credit card has expired or has been cancelled by the issuing bank.
    • If this is true in your case, please update your credit card information.
    • NOTE: You must re-enter your credit card details before we can re-attempt the credit card authorization and process your order.
  • You have moved, or your issuing bank has a different billing address on file.
    • If this is true in your case, please update your billing address.
    • NOTE: You must update your billing address before we can re-attempt the credit card authorization and process your order.
  • There may not be enough room on your card– insufficient credit— for the authorization to go through.
    • If this is true in your case, please contact your bank to clear some credit, or else use a different card. In either case, please re-enter your credit card information.

  • The issuing bank may have declined the charge due to its dollar amount or some other bank validation reason.

Because elevated security checks are in place when you use a credit card, some transactions have trouble with the primary credit card merchant account (payline).

One consistent error received is an AVS error. If you receive an AVS (Address Verification Status) error, it is because Merchant accounts want to see that the BILLING address you are using matches what they have on file. It is important that you mirror the address your credit card company has on your statements. You should then have no trouble.

However, if you continue to encounter errors, send a request to CCO at http://www.cco.us/cco-helpdesk-request/ and ask us to send an invoice from our secondary merchant account (Paypal). When you use your credit card to pay via the link on that email, we often find the transaction will complete.

Please check with your bank if you continue to get an AVS error. This is not a rejection by CCO, it is an inability to authorize from your BANK.

Individuals who make unauthorized copies of software may be subject to civil and criminal prosecution.

CCO makes no warranties, expressed or implied, including without limitation the implied warranties of merchantability and fitness for a particular purpose, regarding the program. CCO does not warrant, guarantee or make any representations regarding the use or the results of the use of the software in terms of its correctness, accuracy, reliability or otherwise. The entire risk as to the results and performance of the software is assumed by you. In no event will CCO, their directors, officers, employees or agents be liable to you for any consequential, incidental or indirect damages (including damages for loss of business profits, business interruption, loss of business information, and the like) arising out of the use or inability to use the software even if CCO has been advised of the possibility of such damages. Because some jurisdictions do not allow the exclusion or limitation of liability for consequential or incidental damages, the above limitations may not apply to you. 5. LIMITED WARRANTY. CCO warrants that (a) the videos will perform substantially in accordance with the accompanying written materials for a period of one hundred eighty (180) days from the date of receipt. Any implied warranties on the SOFTWARE are limited to one hundred eighty (180) days. Some states/jurisdictions do not allow limitations on duration of an implied warranty, so the above limitation may not apply to you.

**CUSTOMER REMEDIES. ** CCO and its suppliers’ entire liability and your exclusive remedy shall be, at CCO’s option, either (a) return of the price paid, or (b) repair or replacement of the program that does not meet this Limited Warranty and which is returned to CCO with a copy of your receipt. This Limited Warranty is void if failure of the videos has resulted from accident, abuse, or misapplication. What’s a browser cache? How do I clear it? Why would I want to?

What’s a browser cache?

The browser cache exists because of a basic assumption made by the folks who design browsers: the internet is slow. Perhaps more correctly, your internet connection is slower than your computer.

What that means is that it’s faster to get something to display from your hard disk than it is to get it by downloading it from the internet. Even with today’s faster internet speeds, that still holds very true.

When current browsers were designed, people noticed that many web pages contained the same elements and many sites had these all over their pages. For example, if you look at this page, you’ll see a logo at the top. It’s actually at the top of every page on this site. So the thinking was why download the same logo for every page? Why not just download it once and then keep it so we can use it again?

The cache is nothing more than a place on your hard disk where the browser keeps things that it downloaded in case they’re needed again. For instance, when you first visit a page on this site, the browser will download the logo so that it can be shown. Every time that you visit a different page on the site, the logo doesn’t need to be downloaded again; as long as the same logo is displayed, it’s already here.

The cache has a limit of how big it can get and you can usually configure how much space to set aside for it. Essentially, when the cache gets full, the items in it that haven’t been used in a while are discarded to make room for whatever new items you’re looking at now.

There’s a little more to it than that. For example, there are ways for me to update the logo on my site and have that override whatever is in your cache so what you see is always up-to-date. But by and large, that’s all it is: a place to keep things locally so that you don’t always have to download the same things all over again.

And it’s all transparent to you.

Until something breaks, of course.

And that’s where “clearing the cache” comes in.

For reasons that I simply can’t explain – other than by saying “stuff happens” – the cache can sometimes get confused. This seems to happen to most browsers and at random times. What you’ll see are partially loaded or badly formatted web pages, incomplete pictures, or in some cases, the wrong picture in the wrong place.

It’s not always a caching problem, but because it happens often enough, “clear your browser cache” is often one of the first diagnostic steps that you’ll hear from people like me.

An empty cache

Your browser cache is empty – so what?

An empty cache means there’s no confusion. As you visit web pages hereafter, the browser will download fresh copies of everything that you see on each page. Effectively, you’ve simply forced your browser to rebuild its cache from scratch as it loads or re-loads web pages. Any cache-related issues should be cleared up.

Is there downtime to my membership access during the late hours of the night?

There are various nightly maintenance processes that run between about 2 AM – 6 AM US Mountain. We usually see very little impact to site performance or availability during this window. On occasion, we will see an instance of database slowdown or downtime, but if it does occur, it is generally no more than a single instance of 1 – 3 minutes.

From the Obliterate the following items from: drop-down menu, you can choose the period of time for which you want to clear cached information. To clear your entire cache, select the beginning of time.

  1. Click Clear browsing data.
  2. Exit/quit all browser windows and re-open the browser.

  1. From the History menu, select Clear Recent History.
  2. If the menu bar is hidden, press Alt to make it visible.
  3. From the Time range to clear: drop-down menu, select the desired range to clear your entire cache, select Everything. 
  4. Next to “Details”, click the down arrow to choose which elements of the history to clear; to clear your entire cache, select all items.
  5. Click Clear Now.
  6. Exit/quit all browser windows and re-open the browser.

  3. Press play and watch the video for a full minute (or as long as possible). Important: Do not let the video end before moving on to step #4 (otherwise, the debug panel will disappear!)
  4. Click the blue “Open link” button.
  5. Copy the link below the words “Share your results by copying this URL.”

System Requirements for Streaming Video

  • Our members’ area offers online streaming videos that are fully compatible with the newest web browsers from Internet Explorer, Firefox, Chrome and Safari. Older versions of web browsers will not work since they do not support HTML5 or modern video files.
  • CCO only supports the Chrome Web Browser which can be downloaded for free here.
  • We recommend a modern computer/laptop/tablet and a high-speed internet connection to experience smooth streaming video playback. Slower computers (pre-2002) and internet connections may result in choppy playback and download errors.
  • Newer iTunes, iPhones, iPads and iPods are fully supported. The iPad 1, unfortunately, is showing its age and doesn’t stream larger videos very well.
  • Android phones are not officially supported, but students report the videos play well on them.
  • Finally, don’t stress the tech stuff too much. Our friendly (and shockingly attractive) support team is always here to help you on your medical coding journey.

In order to watch videos and content in our Member’s Area, you will need a few things.

  1. High-Speed Internet Connection. In order to watch any video content, you will need a speedy internet connection. If you’re not sure how fast your internet is, please visit SpeedTest to run a quick download test. At the bare minimum, you need a download speed of 1.0mbps to watch internet video without experiencing lag and download errors. Most residential high-speed internet providers in the United States meet this requirement. If you’re at work, please keep in mind your company’s technology department might be purposely throttling (slowing down) your internet speed in order to provide consistent speeds for everyone in the company. Finally, please keep in mind that other members in your household might be using up large amounts of download bandwidth if they’re playing video games or watching NetFlix or YouTube.
  2. Current HTML5-Enabled Web Browser. We’re always striving to stay on the cutting-edge of technology and, as a result, we cannot support old browsers such as Internet Explorer 6, 7, 8, 9 or 10. If you’re experiencing problems watching video on an older web browser, please try upgrading to the latest version or try downloading the latest version of Chrome. Our video players are fully compatible with the latest versions of this browser.
  3. Update Flash Player. If you’re using another browser besides Chrome, please update your Flash player to the latest version. You can update it at https://get.adobe.com/flashplayer.
  4. Speedy Computer, Laptop or Tablet. Some of our videos are quite large and can exceed 500mb in size. The process of downloading, playing and “scrubbing” (fast-forwarding) through a video requires a decent amount of memory and processing power. If you’re performing other tasks while downloading and watching the video, the requirement increases. If you’re using an iPad 1 or 2 or a laptop/computer that’s over 7 years old, please try using another computer to see if your problems are resolved. Sometimes the hardware in these older computers simply can’t keep up with today’s popular video formats.
  5. iPad/iPhone/iPod. If you own a newer, speedier device you will be able to play our videos just fine. All our videos are compatible with Apple’s video requirements.
  6. Android: We do not officially support Android phones but students have told us they can play the videos with no problems. If you experience problems with an Android phone, be sure to test alternate Android web browsers for compatibility. All content is delivered as PDF, MP4, M4V or MP3 which are fully compatible with recent smartphones and tablets that support these common file types.

  5. Copy the link below the words “Share your results by copying this URL.”

Education Credits: Questions and Answers

What is an eligible educational institution for education credit:

It is any college, university, vocational school**,** or other post secondary educational institution eligible to participate in a student aid program run by the U.S. Department of Education. This includes most accredited public, nonprofit and privately-owned–for-profit postsecondary institutions.

CCO does NOT participate in the Federal Student Aid program.

A school that wishes to participate in the FSA programs must demonstrate that it is eligible to participate before it can be certified for participation. A school must apply to and receive approval from the Department of its eligibility to participate. Some schools apply only for a designation as an eligible institution (they do not seek to participate) so that students attending the school may receive deferments on FSA program loans, or be eligible for the HOPE/Lifetime Learning Scholarship tax credits or other non-FSA programs that require that the school be FSA-eligible.

The same application is used to apply for both eligibility and certification for participation (see Chapter 2). The Three Definitions of Eligible Institutions The regulations governing institutional eligibility define three types of eligible institutions—institutions of higher education, proprietary institutions of higher education, and postsecondary vocational institutions.

Under the three definitions, a school is eligible to participate in all the FSA programs provided the school offers the appropriate type of eligible program (see chart on next page). This section covers the key elements of the three definitions, giving special attention to those requirements that affect the definition of an eligible program. Although the criteria for the three types of institutions differ somewhat, the definitions are not mutually exclusive. That is, a public or private nonprofit institution may meet the definition of more than one type of eligible institution.

What are qualified expenses for the education tax credits?

In general, qualified expenses for the education tax credits include tuition and required fees for the enrollment or attendance at an eligible post-secondary educational institutions (including colleges, universities and trade schools). The expenses paid during the tax year must be for: an academic period that begins in the same tax year or an academic period that begins in the first three months of the following tax year.

In order to re-certify through AHIMA, credentialed individuals are required to comply with the following continuing education standards as set forth by the Commission on Certification for Health Informatics and Information Management (CCHIIM).

NOTE: At this time, we are only AAPC approved for CEU’s. CCO does not pay AHIMA to get our CEUs pre-approved. However, some attendees of our webinars have told us that AHIMA did accept our CEUs if they asked. CCO cannot guarantee that AHIMA will do this. We suggest that you contact Medical Association of Billers (MAB) and AHIMA & ask them whether you can use the CCO Q&A Webinar CEUs towards their CEU requirements. Only they have the authority to decide on what CEUs they will accept for their organization.

Qualifying Continuing Education Activities and Calculation of CEUs Continuing Education (CE) Content CEUs must be earned within your recertification cycle and must be relevant to the HIIM field as presented in the HIIM domains to be eligible for CE credit. Eighty (80) percent of all CEUs must be earned within the HIIM domain (see Appendix A). The remaining twenty (20) percent can include participation in CE activities on topics not relevant to HIIM.

Calculation of CEUs Calculation of CEUs is based upon clock hours. One (1) CEU is given for each 60 minutes of attendance or participation that is fixed in length. Fractional parts of an hour less than 45 minutes in duration will not be considered a full hour credit. For example, a program that is 2 1/2 hours long is equivalent to two CEUs.

CE Activities 1. Participation in educational programs on topics relevant to HIIM.

a. Educational portions of AHIMA meetings (national convention, state, local, or regional meetings)— one (1) CEU for each 60 minutes of participation

b. Educational portions of programs of allied health and other relevant professional associations—one (1) CEU for each 60 minutes of participation

c. Educational portions of programs sponsored by organizations or vendors on topics that maintain, update, or enlarge knowledge and skills relevant to the HIIM profession—one (1) CEU for each 60 minutes of participation

ICD-10 CEU Requirements

ICD-10 CEU Requirement Extension Update: The Commission on Certification for Health Informatics and Information Management (CCHIIM) has approved an extension of the date by which ICD-10 CEUs can be earned. The new deadline to earn and record the required number of ICD-10 CEUs is March 31, 2016. As the original policy requires, affected credential(s) will be placed in “inactive” status following your original deadline – December 31, 2015 or January 31, 2016. During the inactive period, individuals may earn and record additional ICD-10 CEUs in order to satisfy the requirement. Please view the Extension notification for further details.

This short five-minute video provides information about AHIMA’s ICD-10 continuing education unit (CEU) certification requirements and addresses the CCHIIM ICD-10 CEU Policy, how many ICD-10 CEUs are required to record, how to view CEUs reported in past recertification cycles, and what resources count toward ICD-10 CEU credit.

Recertification Checklist:

  1. Obtain the required number of CEUs during your recertification cycle.
  2. Complete your mandatory annual coding self-reviews (self-assessments), if applicable.
  3. Participate in accepted and qualified CEU activities.
  4. Report your CEUs on time.
  5. Document your CEU activity for auditing.
  6. Ensure your recertification fee is paid.

CEU Requirements for Single Credential

Here are the number of CEUs you must earn for a single credential:

  • RHIA, CHPS, CHDA, CDIP – 30 CEUs
  • RHIT, CHTS-CP, CHTS-PW, CHTS-IM, CHTS-IS, CHTS-TS, CHTS-TR – 20 CEUs
  • CCA, CCS, CCS-P – 20 CEUs, including two mandatory annual coding self-reviews**
  • Physician Coding Specialist (PCS) – 20 CEUs, including two mandatory annual coding self-reviews**
  • Certified in Healthcare Privacy (CHP)* – 30 CEUs
  • Certified in Healthcare Security (CHS)* – 30 CEUs

* AHIMA no longer offers these certification exams.

** Each mandatory annual coding self-review is worth five (5) CEUs toward the total CEU requirement of a coding credential.

CEU Requirements for Multiple Credentials

If you have more than one AHIMA credential, you must earn ten (10) additional CEUs per credential during your two-year recertification cycle. These CEUs may not be duplicated or used for more than one credential.

Here are the required number of CEUs for multiple credentials:

  • CCS and CCS-P – 30 (20 + 10)
  • RHIA with CCS and CCS-P – 50 (30 + 10 + 10)
  • RHIA with CHPS – 40 (30 + 10)

As of Oct. 1, 2015, Medicare, Medicaid, and commercial payers will require providers to submit claims in the ICD-10 format. However, both providers and payers will still have a need to use ICD-9 after the deadline, such as in the following situations: Historical Claims Analysis — Coding professionals — as well as payer claims analysts — will need to use mapping tools to translate ICD-9-CM and ICD-10-CM and PCS codes to make meaningful comparisons between diagnoses, procedures, reimbursement, and other issues that took place before and after the October deadline. These analyses may also include the evaluation of claims for case management, outcomes, utilization trends, and population health.

Identifying High-Risk Codes

Today’s trend analytics are based on ICD-9-CM codes and will need to be translated into ICD-10-CM and PCS codes. The translation process can be challenging, since a single ICD-9-CM code might correspond to several ICD-10-CM or PCS codes. HIM departments will need to work with medical management and disease management to determine the applicable codes, or greatly expand on the codes that trigger alerts.

Provider/Payer Contracting

Currently, provider contracts with payers are structured using ICD-9-CM codes. These contracts will require renegotiation using ICD-10-CM and PCS codes after the October 2015 deadline, and possibly before the deadline in cases where the contract expires before October. Providers will likely need to enlist the expertise of outside consultants to help assess and navigate contract negotiations and payer management.

Coding professionals will need to work with other departments within their organizations to assess contract changes based on the differences between ICD-9-CM and ICD-10-CM and PCS. Particularly important is the analysis of new value-based purchasing or incentive-based contracts that place provider reimbursement at risk depending upon the achievement of patient outcomes and other quality measurements. Accurate code mapping between ICD-9-CM and ICD-10-CM and PCS will help providers negotiate fair contract terms.

Working remotely CPC/COC is often a much more popular combination only because most billing organizations prefer to have their staff on-site. However, the coder doubles as the biller in some organizations, so there is some opportunity there. However, the credential combination we currently see offering the most remote opportunities is the CPC/CRC combination as risk adjustment plans are increasing in market share.

Is it a good idea to get another credential soon after getting the CPC credential? Is it a good idea to take a credentialing course prior to obtaining another credential?

A. Additional credentials may be beneficial in a number of ways including negotiating salary for a position requiring skill sets encompassed by several credentials. So, in my opinion, it’s never too soon to start studying for that next credential. Yes, it is always a good idea to take a full course related to a desired credential before sitting for the said credential, particularly if you don’t have experience in that area.

It depends on the type of coding you are doing.

The COC™ credential indicates you are proficient in coding for outpatient hospital services (outpatient/same day surgeries, outpatient therapies – PT/OT/Speech, etc.) and is very similar in content to the CPC® exam. It includes CPT®, HCPCS and ICD-10-CM.

The CIC™ credential indicates you are proficient in coding for inpatient hospital services (surgical procedures and hospital admissions) as well as the related areas (DRG assignment, payment methodologies, exception payment calculation, documentation regulations, etc.). It is similar in content to the CCS exam offered by AHIMA, and includes ICD-10-CM and ICD-10-PCS codes. However, unlike the CCS, it does not currently include CPT® and HCPCS coding. 

For some facilities, inpatient and outpatient services may be coded by the same group of coders. However, for many facilities, they are handled by separate hospital areas/departments.

Since Risk Adjustment is a retrospective process, CRC coders will still be using ICD-9-CM until 2017.

The HIPAA transaction and code set standards only apply to electronic transactions conducted by covered entities. Other entities, such as employers or casualty insurance plans, may decide to use them voluntarily but are not required to do so. However, CMS will not maintain ICD-9-CM after ICD-10 is implemented.

Because ICD-9-CM will no longer be maintained after ICD-10-CM/PCS is implemented, it is in non-covered entities’ best interest to use the new coding system. The increased detail in ICD-10-CM/PCS is of significant value to non-covered entities. The Centers for Medicare & Medicaid Services (CMS) will work with non-covered entities to encourage them to use ICD-10-CM/PCS.

Non-covered entities such as worker’s compensation, disability and auto insurers that are not required to switch.

Generally, if the non-covered entities don’t want to switch to ICD-10 coding, they don’t have to and can require healthcare providers to submit medical claims with ICD-9 codes.

Because some of the guidelines have changed and the CPC Exam is based on testing your understanding of the guidelines, learning what changed for 2017 will be helpful, in general.  That said, you could delay Updates Webinars until later in your preparation by purchasing the CPC Blitz when it is updated for 2017.  Also, be sure to do timed practice exams because that is critical in adequately preparing for the exam.

Updating only occurs should a guideline or code affect the way we teach coding or questions in quizzes & exams.

Your manuals should match the year you plan to take your AAPC certification exam.  

  • If you’re taking your exam in 2016 use 2016 manuals.  
  • If taking your exam in 2017 use 2017 manuals.  

You are allowed to use whatever year you want but why risk getting a question wrong and failing because of having an outdated manual – our advice don’t do it!

Q: How many CEUs do you need to maintain the CPB? Are you able to do the CPC CEUs to cross those over

A: If you have just a CPB certificate, you need 36 CEUs every 2 years. The CEU certificates will say that it meets the AAPC guidelines for Core A continuing educational units.  If you have a CPB and another core credential such as the CPB, you need 40 CEUs every 2 years.      The CEU requirements are written here: https://www.aapc.com/medical-coding-education/help/.  For people who have other types of AAPC credentials, please be aware of special requirement which are listed for certain credentials. For those special credentials, the certificates will list the special credentials for which it applies.

Q: If I am a student and have not taken the CPC exam yet, should I use Findacode or just stick to my manuals because that is what I will be allowed to use in the CPC exam.  I am trying to do everything right now as it would be on the CPC exam so I don’t develop any bad habits.

A: Students preparing for exams should use their coding manuals to prepare as AAPC only allows manuals for exams.

Findacode is a great coding tool, but will not be helpful in an exam setting as only coding manuals are allowed.

It is not necessary to use an encoder while you are learning and practicing for the CPC exam.  I agree that your focus should be to develop your “book skills” for the exam at this time.  Encoders such as findacode.com require a fee for a subscription so you might as well wait until your employer will pay for it, rather than you paying out of pocket. 

The best feature of using findacode as a student are the explanation of procedures in laymens terms and seeing how the index is used to arrive at a code.

I was an Occupational Therapist and a Therapy Practice administration before I got into coding so I had lots of clinical and documentation experience.  I did a week long boot camp and then a month of studying and then a week of cramming (how the Blitz system was born) and sat for my exam.  So basically outside of my clinical training my focused coding study was about 8 weeks.

Did you miss a CCO live webinar and wish to see it again? 

We understand that sometimes your schedule is too hectic and might be conflicting with our webinar dates either due to work and other personal matters. However, we don’t want you to get behind the topics that are necessary for your preparation for your future career in the medical coding field. So we are giving everyone the opportunity to access the webinars that they either missed or would like to view again anytime through our CCO Club. It also allows you to earn CEUs after taking the CEU quiz even if you missed the live webinar!

Who can join the CCO Club?

Everyone is welcome to join the Club, no matter what level of coding expertise you currently have. We know that at this point, you are feeling anxious about joining the Club. You may be worrying that you might get overwhelmed with the content because you are a newbie in the medical coding field. Well, you don’t have to worry at all. It may be easier for those with medical coding knowledge to understand and grasp the content, but the more you are exposed to helpful coding examples, guidelines, and topics about the medical coding professions, the sooner you will learn it even if you’re a beginner. Therefore, please feel welcome to sign up for the Club.

What will you get from joining the CCO Club?

It provides you the webinars’ full video recording, the full transcript, answer sheets, and a CEU quiz. You must be credentialed before utilizing the CEUs, but anyone can take the quiz to test their knowledge. 

The initial live presentation of the Monthly Q&A Webinar is free, and a service CCO gladly offers to help coders in understanding a variety of topics. We then record each monthly webinar and archive them to our Club, which is open for further viewing exclusively for our Club members.

The CCO Club is how we offer the recorded version of our monthly Q&A webinar, as well as the slide handouts, answer sheets, and full transcript. If you wish to join the Club, you’ll get an access to all previous years of the club. You can go back 12 months and earn a CEU credit for each month by taking a short quiz. Upon successful completion, you will be given with an AAPC CEU certificate.

Does the Club go over the webinar from beginning to the end, including your Q &A?

There are video clips for each instructional segment of the webinar and the chat questions that are answered during the webinar. There is a transcript for the whole webinar, start to finish, plus the answer sheets that each presenter used while presenting, if used. There is also a CEU quiz available so you can still get a CEU if you missed the webinar. When you join the Club, you also get access to all available previous months’ webinars’ resources.

If you’re signed up to Facebook Live Event Notifications with CCO, you’ll receive a short Text Message every time we go live. Inside the Text Message is a simple short link that redirects to our Facebook home page at https://www.facebook.com/cco.us.

To access the FB Live event, click the short link and scroll down until you see the event. Then, click the “Play” button on the FB Live Event video. If you don’t see the Play button or event, some students have reported they need to refresh the page several times until the video and button appear.

If that still doesn’t work, open the browser on your device (Chrome, Safari, etc) and bookmark/save our Facebook page at https://www.facebook.com/cco.us. When you receive the Text Message Alert, simply go to our page on your browser and click on the “Videos” link. The FB Live Event will always appear on that page near the top.

Subject Matter Expert (SME)/Researcher – Intern Position Description

 

Title: Subject Matter Expert/Researcher

Purpose: Develop content that supports learning objectives for educational products and/or job tasks identified by the bodies of knowledge for certification/CEU products

Key Responsibilities:

  • Create nuggets for CCO Club and/or courses
  • Demonstrate ability to navigate and use Intern Nugget and Intern Project applications within the Podio Intern workspace
  • Contribute core content and original material by using Pearl Template in Google Drive and provide a completed document based on Google research
  • Supply source materials and provide links for all resources used
  • Provide links to any other quality resources found in the public domain related to given topic that may be shared with students
  • Incorporate appropriate graphics based on research, ensuring that they are based on CCO’s policy on usage rights and creative commons
  • Write multiple-choice review questions to go with completed documents
  • Meet all agreed-upon turnaround time for projects

 Qualifications:

  • Knowledge of medical coding
  • Excellent writing skills and attention to detail
  • Ability to communicate in a professional, concise manner while at the same time providing enough information that will leave few unanswered questions
  • Capacity to see a project through to completion

Time Commitment:

  • The time commitment may vary by project. The scope will be established (and reviewed with the Subject Matter Expert) at the onset of the project

Training/support provided:

  • Project orientation video
  • Guidance on the development process
  • Conference calls or online development meetings as necessary
