Q&A Anemia on PT account

Hello all!!! I am excited to be a part of the group and would like to thank you all in advance for helping me as I learn... maybe one day I can help someone else :) My question is regarding coding anemia. I am currently coding physical therapy. Anemia was listed in the PMH with no other mention in the report; as well, labs were not available for this visit since it was a PT visit. Should the anemia (NOS) be coded, since it was not backed up as being an active diagnosis?

I would only code the DX for what the Physical therapy was prescribed for. Id follow up with the Ordering if they did not provide a proper DX (since PT's can't diagnosis)
