Q&A Diabetes with hyperglycemia (E11.65) vs "Generic" diabetes (E11.9)

Hey everyone!

With the "unfreeze" deadline of Oct 2017 for ICD-10-CM, I am trying to prepare. If a patient is documented as having type 2 diabetes and the physician documents in another place a glucose level of 258 (just an example), could this be coded to E11.65 (type 2 diabetes with hyperglycemia) or would it have to be coded to E11.9 (type 2 diabetes without complications "aka generic code for diabetes")?

Let me take this one step deeper... If a patient has type 2 diabetes and is on a sliding scale insulin protocol but the patient's glucose is at a level where the physician would like to be called and he/she gives an order for a specific amount of insulin to be given can this be coded to E11.65? (This would be based on the physician's order for additional insulin units...and the physician never mentions this in his/her progress notes or discharge summary)

I have read the ICD-10-CM 2017 Chapter Specific Guidelines and it does not address it just like the 2016 didn't.

How are your physicians documenting/how are you coding to try to get away from a "generic" code of E11.9?
