When giving a vaccine for exposure to rabies, do you need to use both the z23 and z20.3 dx codes? We would be using 90675 as the CPT code. Would the same hold true for exposure to other communicable dx as well?

The most important code is the Z20.3 for medical necessity of the rabies vaccination. The diagnosis code, Z23, encompasses all vaccinations at this time. However, Z23 is to be used when the primary reason is an encounter for immunization. It can be used as a secondary code if the inoculation is given as a routine part of preventive health care. The rabies vaccine and visit is not routine, so according to the guidelines the Z23 would not be needed.

If you are billing for Medicare. Z23 will get most claims denied since they don't cover routine Vaccines. As specified by Melissa above, code the reason of the vaccine is needed if its not routine.
