CPC Renewed my CPC, but being audited

I just had my very first CPC certificate renewed a little over a month ago. Unfortunately, I am being audited.

I was a member of Reply Club for a month, and got half of my CEUs there (it's awesome!). I did save the certificates after the quiz, but I just found out that "my name" and the "date" isn't on the certificate. I called AAPC. The representative wasn't sure if that's ok. She just told me to send in whatever I have, and they will decide. I don't want to risk it so come here for help. Do I leave the name and date blank as it is or write my name and the date myself? Any help will be greatly appreciated. Thank you.

You're supposed to write your own name and date in when you get the certificate. Please email https://www.cco.us/cco-helpdesk-request/ if you need help with proof of date you took exams if you didn't keep them yourself.

It is a completely random thing the audits. I was audited my very first year as a coder. The good advice I got from another coder was to put each of my hard copied CEU forms in a notebook. I needed to only pull them out, copy them and fax them in. No trouble. With technology you can now scan them into your computer and e-mail them to the address they give you.
