Use of LT and RT modifiers

Are there any rules of thumb regarding when to use the modifiers for -LT and -RT? For example, according to the answer key for the Workbook exercises Chapter 27 #41 and 43:
69320_LT is used for Myringoplasty of the left ear.
69661 (without _LT) is used for Left stapedectomy with footplate drill out.
This seems to arbitrary to me and I never know when to use these modifiers even though clearly there are Left and Right ear drums and Left and Right stapes.

As Laureen explained in the July 2012 Medical Coding Certification Q & A Webinar, use -LT and -RT whenyou need to distinguish from Left and Right. The use of these modifiers is often based on the payer requirements.
